In infants and young children, the most accurate way to measure heart rate is by auscultating the apical pulse with a stethoscope and counting for a full minute. This site reflects the true number of heartbeats and is less affected by peripheral perfusion or movement than peripheral pulses. Radial pulses can be weak or irregular in this age group because their small peripheral arteries don’t always mirror the actual heart activity, especially when the child is upset or crying. The temporal pulse is harder to locate reliably in infants and can give less dependable readings. The carotid pulse is not ideal for routine checks in babies and young children because it can be difficult to palpate cleanly and can cause distress or vagal responses; it’s better reserved for emergencies when other pulses aren’t palpable. Therefore, the apical site is preferred for accurate assessment up to about age 3.
